G&PE Partner MOU Request Form

INSTRUCTIONS: Please provide as much information as possible regarding your request in the form below.

Step 1:

Have you created the partner record and associated partner contact(s) in Slate? *

Yes

If the partner record has not been created, submit a Slate User Request form, with a request type: Partner ID/Partner Contact ID. Then create a new Partner Record with the ID(s) provided by ESOA. Update any information on the partner record and add any activities to the Interactions/Timeline.

NOTE: You cannot submit this form until the Partner Record has already been created.

Step 2:

Download the MOU template for the type of partnership and discount. Enter the partner name, details, and signer information onto the MOU template you have downloaded. Save the MOU and route for signatures.

Has the MOU been signed by all parties? *

Do you need assistance routing the MOU for the AVP's signature? *

Step 5:

Attach the completed and signed MOU to the partner record within the Materials tab.

Have you attached the final MOU with all signatures to the materials tab of the partner record? *

Yes

If the MOU has already been signed by all parties, please attach the final copy to the Materials tab of the partner record with a material type of "Partner MOU".
